Point-of-Care Diagnostics: Can We? Should We?

For years now, clinical point-of-care testing has been mostly limited to blood glucose testing, drug tests, and  pregnancy tests.
Where are the point-of-care tests for infectious disease, cancers, heart disease, and other diseases where it would seem to be a benefit? Do we lack the technology, or are there barriers to entry that are not yet obvious?
